DECREE
AND NOW, this G?A',Xc day of 2014,. upon consideration
of the Petition for Allowance of Distribution of Principal of an Incapacitated Person, it is hereby
ORDERED and DECREED that Gloria J. Banks, plenary guardian of the estate and person of
her mother, Inez H. Davis, is authorized to spend from the estate of the Incapacitated Person,
Inez H. Davis, as follows:
Such funds as are necessary during a period of hospitalization or inpatient
rehabilitation to reserve the Incapacited Person's bed at the nursing facility in
which she resides;
Such funds as are necessary to hire assistants to provide care (up to 8 hours per
week) or to accompany the Incapacitated Person to health care appointments
when Petitioner or staff from the facility is not able to provide this service.
Legal fees in the amount of$511.11 to SkarlatosZonarichLLC.
