RECEIPT& RELEASE AGREEMENT
This Agreement is made and entered into effective on the date of execution by both
Christine Kanelakis, Executrix ("Executrix") of the Estate of Demetra K. Scoropanos, aka
Demetra L. Scoropanos, aka Demetra Scoropanos, Deceased, party of the first part
And
Pennsylvania Department of Public Welfare ("Claimant"), by its duly authorized
representatives, parry of the second part
WHEREAS, in accordance with their desire that the administration of the Estate of
Demetra K. Scoropanos be terminated without the expense and further delay of a court
accounting, the parties hereto, in consideration of the mutual covenants herein expressed, and
intending to be legally bound hereby, agree that:
1. Demetra K. Scoropanos died on January 19, 2011. Her estate is now in the process
of administration with the Register of Wills of Cumberland County, Pennsylvania having granted
Letters Testamentary to Christine Kanelakis as Executrix on November 16, 2011 at No. 21-11-
1235.
2. Under provisions of the Decedent's Last Will dated October 5, 2005, the Decedent
bequeathed her estate as follows: $500.00 to each of her 12 grandchildren, and the remainder of
her estate in equal shares to her 4 children,per stirpes.
3. The estate is insolvent and there are no assets available for distribution to any
beneficiaries of the estate. No inheritance tax is due or owing to the Commonwealth of
Pennsylvania. The Notice of Inheritance Tax Appraisement issued on September 24, 2012 is
attached hereto as Exhibit"A".
4. The assets of the Estate consist of the following:
a. M&T Bank Account No. 858862 $3,686.08
b. Claremont Nursing& Rehabilitation Center - Refund $2,703.42
C. CareFirst Blue Cross/Blue Shield - Refund $ 248.46
Total $6,637.96
5. Funeral expenses of the Estate consist of the following:

7. Debts of the Estate consist of the following:
a. Class 3 Claim: Commonwealth of PA $31,074.09
b. Class 6 Claim: Commonwealth of PA $201,491.04

8. Pursuant to Section 3392 of the Probate, Estates and Fiduciaries Code, if the
applicable assets are insufficient to pay all proper charges and claims in full, the Executrix,
subject to any preference given by law to the claims of the United States, shall pay claims in the
order as directed in Section 3392, without priority as between claims in the same class. The
Executrix has the sum of$6,697.36 available for payment of claims.
9. The Executrix has paid or proposes to pay the claims against the estate for costs of
administration, in full, as prescribed by Section 3392, Class (1) of Section 3392, in the amount of
$1,718.28, leaving a balance of$4,919.68 for payment of remaining claims.
10. The next class of claims payable is pursuant to Section 3392, Class (3) which total
$33,435.47 and consist of funeral/burial related costs in the amount of$2,361.38, and the costs
of medical/nursing services furnished within six (6) months of the date of death in the amount of
$31,074.09. The Executrix proposes to pro-rate the 2 claims into the balance remaining and pay
$344.38 for funeral/burial claims and $4,575.30 for the medical/nursing services.
11. There are no funds available for payment of the balance of the Class 3 claim of the
Claimant in the amount of$26,486.70 or for payment of the Class (6) claim of the Pennsylvania
Department of Public Welfare in the amount of$201,491.04.
12. The Claimant consents and agrees that it shall accept the sum of$4,575.30 in full
and complete settlement, satisfaction, release, compromise and discharge any and all further
claims, demands, actions or causes of action, legal or equitable, absolute or contingent, vested or
hereafter to accrue, which it may have against the Demetra K. Scoropanos, subject to the
provisions of Article 13 that follows.
13. The Executor agrees to immediately notify the Department of Public Welfare,
Estate Recovery Program in the event that additional estate assets are located in the future for the
Demetra K. Scoropanos Estate. Thereafter, the Executor agrees to make payment of the said
assets to the Department of Public Welfare, Estate Recovery Program, in accordance with the
Pennsylvania Decedents, Estates, and Fiduciaries Code.
14. This Agreement may be executed in multiple counterparts and, when so executed
by all parties hereto, shall be binding upon all the parties, and their respective heirs, next-of-kin,
personal representatives and assigns. Thereafter, this Agreement shall be effective upon receipt
of payment by the Claimant.
IN WITNESS WHEREOF, the parties have hereunto set their hands and seal the day and
year hereafter set forth.
