Martens having a last known address of 118 2 nd Street, PO Box 487 Boiling Springs, PA 17007 ( "Owners ") and all that certain tract of land including improvements thereon, owned by the Owners and described as follows: 3608 Dwayne Avenue Mechanicsburg, PA 17050 Tax Property Map: 10 -18 -1312 Parcel No.: 138 for unpaid sewer and /or trash services rendered from April 1, 2013 through January 31, 2014. Said services were rendered and charged pursuant to Hampden Township Code of LAW OFFICES Ordinances Chapter 18, Part 2 and /or Chapter 20, Part 1, as amended. SNELBAKER E( BRENNEMAN P.C. Attached hereto is a statement of the kind and character of the services so Q.�3a S/7y rendered and the prices charged therefor, said statement being a true and correct copy of the Owners' account with the Claimant, which sum of $2,038.77 was duly assessed against the above Owners and premises beginning April 1, 2013 and for which sum, which includes interest, costs and penalties, together with attorney's fees pursuant to Section 209, Part 2, Chapter 18 of the Township of Hampden Code of Ordinances, a lien is claimed against the above premises in accordance with the Municipality Authorities Act of June 19, 2001, P.L. 287, 53 P. S. § 5601, et seq ., as supplemented and amended and the Municipal Claims and Tax Lien Act of May 16, 1923, P.L. 207, 53 P.S. § 7101, et se ., as supplemented and amended.
