2014. Said services were rendered and charged pursuant to Hampden Township Code of
Ordinances Chapter 18, Part 2 and/or Chapter 20, Part 1, as amended.
Attached hereto is a statement of the kind and character of the services so
rendered and the prices charged therefor, said statement being a true and correct copy of
LAW OFFICES the Owner's account with the Claimant, which sum of$414.54 was duly assessed against

interest, costs and penalties, together with attorney's fees pursuant to Section 209, Part 2,
Chapter 18 of the Township of Hampden Code of Ordinances, a lien is claimed against
the above premises in accordance with the Municipality Authorities Act of June 19, 2001,
P.L. 287, 53 P. S. § 5 60 1, et sea., as supplemented and amended and the Municipal
Claims and Tax Lien Act of May 16, 1923, P.L. 207, 53 P.S. § 7 10 1, et sea., as
supplemented and amended.
