Mild Mental Retardation

Incapacitated Person is receiving:
Socially, Todd attends Special Olympics &Easter Seals events, Carlisle Best
Friends Sports,Therapeutic Riding and Horse Day Camp through Chasing
Rainbows, and is part of a sprint car race team . He receives medical assistance,
SSI, and is in line for a waiver from MH/MR.
V. GUARDIAN'S OPINION
A. It is the opinion of the Guardian of the Person that the guardianship should:

The reasons for the foregoing opinion are:
Todd Jr. has a life-long disability in the form of mild mental retardation,
Autism/PDD-NOS , which significantly makes him totally unable to manage
common sense every day decisions, and for which he needs care basically 24/7.
� During the past year,the Guardian of the Person has visited the Incapacitated Person
times with the average visit lasting hours, minutes.

The report of a social service organization employed by the Guardian to oversee and
coordinate the care of the Incapacitated Person for the period covered by this Report may be
attached to supplement this Report.
