The order was necessary because client's caregiver died, she was being evicted from the house and she was unable to care for herself due to dementia. She was transported to the Holy Spirit Hospital's emergency room for medical evaluation. Upon discharge she was admitted to dementia unit of The Todd Home, Carlisle, PA where she is still residing. At the time of her admission, Ms. Floyd was diagnosed with a urinary tract infection, uncontrolled hypertension and dementia with delusions. On May 15, 2007 Judge Ebert appointed the Cumberland County Office of Aging Permanent Plenary Guardian of Person and Estate for Elizabeth Floyd. Since that time she had also been diagnosed with ambulation dysfunction, atropthy, a history of a vertebrae compression, macular degeneration, osteoperosis, kyphoscoliosis. More recent diagnoses included rheumatoid arthritisneuromyopathy, Korsakoff syndrome, dysarthia depression, NIDDm, afib, CAD, and GERD.she does suffer from occasional urinary tract infections. Elizabeth began experiencing a slow decline. Her weight dropped from 190 to 177 but this was a healthy weight. She ate about 44% of her pureed meals. She also had 4 supplements daily along with 4 oz. of ice cream after her evening meal. She needed to be fed by staff because of her failing eyesight. In August of 2013 she experienced a non-responsive episode. When she came out of it she was talking but her blood sat level was low so they put her on oxygen. The oxygen was at 5 liters and as soon as they take it off the blood sat starts to go down so she was on it all the time for about a week. During this time, the fact that she had no advanced directive came out. We discussed this with Dr. Branscum who is both our physician consultant and also Elizabeth's personal physician who sees her in the nursing home. He said she is experiencing hypoxia and probably just stopped breathing. It could have been cardia, vagovasal or intercranial. I was initially unable to get in touch with the daughter because the phone number we had for her was disconnected. I sent a letter to the last known address and requested she contact us which she did. I explained the circumstances and she agreed to comfort measures only. I advised the nursing home accordingly. On March 8 of this year she developed a urinary tract infection that could only be treated by antibiotics through IV so we gave permission for this. We were advised the next week that she was doing better. On March 17, 2014 we received a call from the charge nurse stating that she was non- responsive at this point. She was septic and started to have mottling. I asked her what the cause was and she said she would call Dr. Branscum. She called back and said that he said her body is just wearing out and that he would be in agreement with palliative care. I agreed to palliative care. I called the daughter and she also agreed to palliative care. She passed away on March 18, 2014. The nursing home advised the family. Funeral arrangements had previously been made with Hollinger Funeral Home. The graveside service was on April 8, 2014. Three staff from the Office of Aging, her two sons and wives and a friend were in attendance. The service was officiated by the chaplain from Sarah Todd Home where she resided. ESTATE A guardianship account was maintained for Elizabeth at M&T Bank. The balance of her account at the time of her death was $4951.19.
