PETITION FOR THE APPOINTMENT OF GUARDIAN OF THE PERSON OF
YADIRAH SHAMSID-DEEN, A MINOR UNDER THE AGE OF FOURTEEN YEAR~
PURSUANT TO 20 PA. C. S. SECTION 5111 (a) "

The Petition of Harvey R. Hoover and Lena S. Hoover respectfully sets/forth tAlit:
1. Your Petitioners, Harvey R. Hoover and Lena S. Hoover (hereinafter referlid to
as "Petitioners"), are sui juris Caucasian adults of Mennonite religious affiliation, who
have resided in Cumberland County at 1335 Ritner Highway, Shippensburg, PA 17257,
since about January, 1992.
2. The above named female child is 3 years and 11 months of age, having been
born on March 8, 2001, in Temple University Hospital, Philadelphia, PA.
3. The birth mother of the child is Nadiyah Shamsid-Deen, whose last known
address is 2765 North Hemberger St., Philadelphia, PA 19132.
4. Petitioners do not know the identity or address of the birth father of the child.
5. The child has resided with petitioners at their above-stated address from March
10, 2001, until about January, 2002, at which time the birth mother took custody of the
child for about one month, and then the birth mother asked petitioners to resume the
care and take custody of the child, which they did at their above-stated address in
about February, 2002, until the present.
6. Petitioners received custody of the child from the birth mother.
7. Upon information and belief, the birth mother was incarcerated at Philadelphia
Institution of Correction Center about the time she gave custody of the child to
petitioners, and was serving a prison term of 18 months, however, she was released
about ten months after she gave custody of the child to petitioners.
8. Petitioners learned about the situation of the birth mother through the Catholic
Diocese of Philadelphia.
9. The birth mother executed a document dated March 14, 2001, purporting to grant
temporary guardianship of the child to petitioners for one year, a copy whereof is
attached hereto as Exhibit A.
10. At all times during which petitioners have had custody of the child, the birth
mother has not provided care or support for the child, though the child's grandmother
did send two separate packages of clothes and some toys. Nor has the birth mother
during that same time period visited the child at petitioner's residence; however,
petitioners did bring the child to visit the birth mother once per month during the
incarceration of the birth mother.
11. Primary care of the child has and is being provided by the petitioners.
12. It is necessary under the foregoing circumstances for the petitioners to be
appointed as guardians of the child.
13. No other guardian has been appointed for the child.
14. There is no other litigation pending regarding the guardianship of the child.
15. The proposed guardians have no interest adverse to the child.
16. The child does not own any property and does not have any income.
17. Petitioners do not know the religious affiliation of the birth parents of the child.
WHEREFORE, the petitioners requests that this Court appoint them as guardian of
the person of Yadirah Shamsid-Deen, a minor under the age of 14 years, in
accordance with 20 Pa. C. S. Section 5111 (a), until such time as either one of the birth
parents of said minor apply to the Court and show that she, he, or they are ready,
willing, and able to take care, custody, and control of said minor.

CONSENT OF PROPOSED GUARDIANS
The undersigned, Harvey R. Hoover and Lena S. Hoover, of 1335 Ritner Highway
Shippensburg, PA 17257, hereby give their consent to the proposed appointment by
this Court ofthemselves as guardians of the person ofYadirah Shamsid-Deen, a minor
under the age of 14 years, in accordance with 20 Pa. C. S. Section 5111 (a).
