DeLONE, minors ORPHANS COURT DIVISION GUARDIAN OF MINOR'S ESTATE 2\ - 0'2. - 4'-\0 PETITION FOR APPOINTMENT OF A GUARDIAN OF MINORS AND NOW come J. Bret DeLone, M.D., and Carrie L. DeLone, M.D., by their attorney, Samuel L. Andes, and petition the Court to appoint a guardian of the estate of their minor children, based upon the following: 1. The Petitioners herein are J. Bret DeLone, M.D., and Carrie L. DeLone, M.D., adult individuals who reside at 320 North 25th Street in Camp Hill, Cumberland County, Pennsylvania. The Petitioners are married and are the natural and legal parents of Alexandra DeLone, Victoria L. DeLone, and Charles P. DeLone, all of whom are minors and who are the subject of this petition. 2. The minors for whom this petition seeks appointment of a guardian are: A. Alexandra DeLone, a female child born on 17 March 1990, who resides with the Petitioners at 320 North 25th Street in Camp Hill, Pennsylvania. B. Victoria L. DeLone, a female child born on 31 January 1993, who resides with the Petitioners at 320 North 25th Street in Camp Hill, Pennsylvania. C. Charles P. DeLone, a male child born on 15 August 1996, who resides with the Petitioners at 320 North 25th Street in Camp Hill, Pennsylvania. 3. It is necessary that a guardian of the estate be appointed for all three minors because they received an interest in financial assets as a result of the death of their grandfather, Pasqual A. Donvito, who had designated them as beneficiaries to receive such accounts. II 4. No other guardian has been appointed for the estate of any of the minors. 5. The proposed guardian, Carrie L. Delone, is 44 years of age, resides at 320 North 25th Street in Camp Hill, Cumberland County, Pennsylvania. She is the mother of the three minors and has no interest adverse to them. Her consent to serve as guardian is attached hereto and marked as Exhibit A. 6. Each of the minors has an interest in financial accounts held with the Teachers Insurance Annuity Association - College Retirement Equities Fund which has a current balance of approximately $8,500.00 and which is currently held by that fund pending appointment of a guardian. 7. None of the minors have any income except the anticipated income from the accounts within the Teachers Insurance Annuity Association - College Retirement Equities Fund, which petitioners estimate to be no more than $350.00 per year. WHEREFORE, Petitioners pray this Court to appoint Carrie L. DelaNE, minors ORPHANS COURT DIVISION GUARDIAN OF MINOR'S ESTATE CONSENT OF PROPOSED GUARDIAN The undersigned, Carrie L. Delone, M.D., hereby consents to act as guardian of the estate of Alexandra Delone, Victoria L. Delone, and Charles P. Delone, minors, and states, in support of the attached petition, the following: 1 . I reside at 320 North 25th Street in Camp Hill, Cumberland County, Pennsylvania, and am employed as a physician by ~~ \ ("\. q ~'S ,\Q..\.()..(\S \I')C- . 2. I am a citizen of the United States and can speak, read, and write the English language. 3. I am not the fiduciary or an officer of, or employee of, any corporate fiduciary of any estate in which the minor has an interest, or the surety or an officer or employee of the corporate surety of such a fiduciary. 4. I have no interest adverse to any of the minors. 5. It is not my intention to apply for an allowance of support or education of any of the minors during their minority.
