Plaintiff, Citizens Bank of Pennsylvania, is a business organization duly organized and existing under the law of the Commonwealth of Pennsylvania, with its principal place of business located at the address above-captioned. 2 . Defendant, Richard C. Thomas, is an adult individual residing at the address above-captioned. 3 . The defendant, for valuable consideration received, executed and delivered to plaintiff a promissory note under the terms of which the defendant promised to pay to the plaintiff consecutive monthly payments under the terms and conditions set forth in the promissory note in the form of a Home Equity Line of Credit Agreement . A true and correct copy of the aforesaid Home Equity Line of Credit Agreement is attached hereto, made a part of this complaint and marked Exhibit "A" . 4 . Contrary to the terms of the aforesaid Home Equity Line of Credit Agreement, the defendant failed to make the required payments when due as a result of which the unpaid balance of $203, 446. 03 became due and payable. 5 . All the credits to which the defendant is entitled have been applied and there remains a balance due of $203, 446. 03 . 6 . Plaintiff has made demand upon the defendant for payment of the balance due but the defendant has failed and refused and still refuses to pay the same or any part thereof. 7 .
