PURSUANT TO 20 Pa. C.S. & 3155
PRESBYTERIAN HOMES, INC. D/B/A GREEN RIDGE VILLAGE ("Petitioner"), a
principal creditor of DORIS R. WIDRA, respectfully represents that:
1. DORIS R. WIDRA ("Decedent") died on December 2, 2013. An original Death
Certificate is attached hereto as E�ibit"A".
2. Upon information and belief and to the extent of Petitioner's knowledge,
Decedent died intestate.
3. Upon information and belief and to the extent of Petitioner's knowledge,the
Decedent has the following surviving heirs:

4. At the time of her death, Decedent was a resident of Petitioner's slcilled nursing
facility located at 210 Big Spring Road,Nevwille, Pennsylvania 17241.
5. Decedent currently owes an outstanding balance to Petitioner for goods and
services that it provided to Decedent prior to Decedent's death.
6. Petitioner desires to have Shaun O'Toole, Esquire, The Elder Law Firm of Shaun
O'Toole, 220 Pine Street, Harrisburg, PA 17101, (717) 695-0389, appointed by the Court to
administer the Estate of DORIS R. WIDRA, for the purpose of paying all debts owed by
Decedent and distributing the balance of the estate pursuant to the intestate laws of the
Commonwealth of Pennsylvania.
WHEREFORE, Petitioner, PRESBYTERIAN HOMES, INC. D/B/A GREEN RIDGE
VILLAGE, respectfully requests that a Citation be issued pursuant to 20 Pa. C.S. § 3155, to
show cause why Letters of Administration for the Estate of DORIS R. WIDRA should not be
issued to Shaun O'Toole, Esquire, The Elder Law Firm of Shaun O'Toole, 220 Pine Street,
Harrisburg, PA 17101, (717) 695-0389.
