1. The attached Special Needs Trust under 42 U.S.C. § 1396(d)(4)(A for the
benefit of Claire Wong Crego is created, approved, and incorporated herein by
reference;
2. Geoffrey B. Crego is hereby appointed to serve as Trustee of the said Trust
pursuant to the terms of the Trust;
3. The Trustee shall accept the funding assets and shall hold and administer the
funds in accordance with the terms of the said Trust.
