AND NOW, this day of (�,� , 2014, upon consideration of the

Petition by Cumberland County Aging and Community Services For Appointment of Plenary
Guardians of the Person and Estate of Joan S. Grove and the hearing held on the Petition before
this Court, the Court finds, based upon clear and convincing evidence,that Joan S. Grove is an
incapacitated person in that she suffers from mental impairment that prevents her from receiving
and evaluating information effectively and prevents her from formulating and communicating
decisions to such a significant extent that she is unable to manage her financial affairs and meet
the essential requirements for her physical health and safety on a daily basis. Accordingly,
Cumberland County Aging and Community Services is hereby appointed as plenary guardians of
the person and estate of Joan S. Grove.
Cumberland County Aging and Community Services shall serve without bond and is
permitted to receive compensation for its services as plenary guardians in the amount of$100 per
a cn �nth or ten�rcent(l 0%) of Joan S. Grove's gross monthly income, whichever is less.
