OF A MINOR CHILD UNDER THE AGE OF FOURTEEN YEARS
1. The minor is Jake Nicholas Sherman, age 1 month, who resides at 32 Central
Blvd., Camp Hill, PA 17011. The mother of the minor is Mallory Fern Sherman, who
resides at 32 Central Blvd., Camp Hill, P A 17011. The father of the minor is not known
conclusively.
2. The petitioner is Theresa L. Sherman, who resides at 32 Central Blvd.',Camp

Hill, P A 17011 and who is the grandmother of the minor child.
3. The minor child's mother consents to the petition.
4. The appointment of a guardian is necessary in order that the minor child can
quality to be covered under the health insurance plan in which the grandparents are
enrolled. No other guardian has been appointed for the person of the minor.
5. The proposed guardian is Theresa L. Sherman, age 47, who resides at 32
Central Blvd., Camp Hill, P A 17011, and is the grandmother of the minor child.
6. The proposed guardian has no interest that is adverse to the minor.
7. The religious persuasion of the mother of the minor child is Catholic. The
religious persuasion of the proposed guardian is Catholic.
8. The petition is not for the appointment of a guardian of the estate.

9. The minor is not entitled to receive any money as a party to any action or
proceeding in any court.
10. The minor is not the child of a veteran.
11. The proposed guardian's written consent is attached to this petition as Exhibit
"A".
WHEREFORE, Petitioner respectfully requests that this court appoint Theresa L.
Sherman as guardian of the person of Jason Nicholas Sherman, a minor.
