GAUL (hereinafter referred to as "Grandmother") of Cumberland County, Pennsylvania, having reached an agreement to mutually affirm a custodial arrangement and appointment of guardian of their minor child, ERIN RAE GAUL, born November 12, 1985, desire to legally obligate themselves to adhere to the terms of said agreement; NOW, THEREFORE, it is hereby agreed and stipulated by and between Thomas J. Gaul, Pamela J. Gaul and June S. Gaul, as follo\NS: 1. We, Thomas J. Gaul and Pamela J. Gaul, natural parents of Erin Rae Gaul, hereby appoint June S. Gaul, of 1904 Letchworth Drive, Camp Hill, Cumberland County, Pennsylvania as guardian and/or legal custodian of our minor child, Erin Rae Gaul. 2. We agree that the appointment of June S. Gaul, as guardian and/or legal custodian or our minor child authorizes her to act on the behalf of the minor child in accordance with all of the rights and duties conferred upon said guardian in accordance with the laws of the CommonVYe8lth of Pennsylvania. 3. Further, it is our intent that our daughter, Erin Rae Gaul, fully reside with 1 her grandmother, June S. Gaul and that June S. Gaul hereby assumes all parental responsibilities, including all decisions regarding the minor child's enrollment in the school district vmerein June S. Gaul resides. 4. This Agreement may be modified at any subsequent time by mutual consent of the parties, reduced to writing. 5. This Agreement contains all the understandings, promises and covenants between the parties with regard to the custodial arrangements for Erin Rae Gaul and guardianship of said minor child. 6. The parties further stipulate and agree that this custody and/or guardian stipulation may be submitted at any time to the Court of Common Pleas of Cumberland County, or a court of proper jurisdiction, with request that the court approve this Agreement and Stipulation, and enter the same as an order of court.
