Prior to this appointment Mrs. Ceja resided in her own apartment at 1108 Yverdon Drive Apt. C4 Camp Hill, Pennsylvania. She was admitted to the Forest Park Health Center Carlisle on May 27, 2005 after the outcome of an Emergency Guardianship hearing. Mrs. Ceja continues to reside at Forest Park Health Center. She was moved from the dementia unit to the nursing unit in August 2007. Due to her severe cognitive loss she was no longer benefiting from the activities and structure of the dementia unit. She continues to be diagnosed with senile dementia Alzheimer's type, depression, Parkinson's, PVD, osteoporosis, hypertension and anxiety. Her medical and cognitive status are stable. She continues to be nonverbal as well as non-ambulatory. The staff at Forest Park Health Center provides 24 hour supervision, maximum hands on assistance with activities of daily living. Theresa is a 2 person assist with the Hoyer lift for all transfers. Because she is a high risk for skin breakdown she has a pressure relief cushion on her wheel chair. In her bed she has a foot cradle to alleviate toe and heel pressure and is repositioned every two hours. She receives restorative therapy which includes two splint programs to both upper and lower extremities with the goal to decreae or prevent further contractures. She contines to eat 75-100% of her pureed food meals and enjoys the ice cream supplement she is given. Her weight is stable. She is a passive participant with activities. She is taken to observe but she is unable to participate nor does she show any interest. She was admitted to hospice services through Heartland Hospice. They provide assistance with feeding at lunch and companionship. She sees a dentist and a podiatrist on an as needed basis and her doctor on a routine basis. When visited keeps her eyes shut most of the time. She shows no distress and is reactive only to touch stimulation. In addition to visiting client, care plans are attended every 90 days and the medical record is reviewed and discussed with the nursing staff. The staff at the nursing home provides updates and notifications of any changes that Theresa may have experience. It is the request of the Cumberland County Office of Aging that the Agency retains Guardianship of Person and Estate of Theresa Ceja. Forest Park Health Center has demonstrated an adequate quality of care for client by managing her daily needs and continuously monitoring her medical conditions. Mrs. Ceja's appearance is always good and she appears to be content in her present living situation. Therefore, we believe that Mrs. Ceja should continue to reside at Forest Park Health Center. ESTATE Funeral arrangements were made with Hollinger Funeral Home in Mt. Holly. Mrs. Ceja has an irrevocable burial trust with Sovereign bank. Currently her income includes $225.44 from a state annuity, $160 civil service and $720 from Social Security. Her payments to Forest Park are currently $1058.44. The Office of Aging also receives $100 a month for a Guardianship fee as provided for in the guardianship court order. Christmas gifts were donated by volunteers with Home Instead, a home health agency. A summary of income and expenses and check register are attached.
