On February 20, 2012 an emergency Guardianship hearing for Patricia 1/,�b��F�on w�s :��- —"�
held before Judge AI Masland. Cumberland County Aging and Communit �ervic�wa� m
appointed her Emergency Guardian of Person and Estate. At the time �he hea�� , � �
Ms. Woolfson was a patient at Holy Spirit Hospital. She was admitted following a fal1.
When EMS arrived she was found on the floor in water that had overrun the sink
because the faucet was on. She was found to be hypothermic and had open sores on
both legs and "overpowering stench." Her open wounds needed surgical debridement
or else she might have died. She refused the surgery and a psychiatrist determined her
to be incapacitated. Following the hearing, the Office of Aging approved the surgery.
She was later admitted to Golden Living Center-Camp Hill where she still resides.
On March 12, 2012 the final guardianship hearing was held and Cumberland County
Aging and Community Services was appointed her Permanent Plenary Guardian of
Person and Estate by Judge AI Masland.
PERSON
Following the hearing Ms. Woolfson was returned to Golden Living Center-Camp Hill
where she continues to reside.Her admitting diagnoses were anemia, HTN, GERD,
diverticulosis, anxiety, GI bleed and Iron deficiency.
Patricia is rather difficult and argumentative, often refusing personal care. She does
come out to the hallway, eats in the dining room and goes to activities but only
observes, does not participate. She remains a 2 person assist with the lift but once she
is in the wheelchair, she is able to self-propel the wheelchair. She continues to display
anxiety, agitation and paranoia.
In September 2013 on two occasions, she refused to see a cardiologist. Her health
seemed to decline during finro months prior to this time. After consulting with our
solicitor, an Aging staff member talked with her about it then made another
appointment. She agreed to go. She later told the staff member that she did not want to
go because one time a cardiologist massaged her heart with dirty hands and her heart
rate went up because of it.

On December 26, 2013 she was taken to the hospital because she had developed
respiratory distress and was placed in Intensive Care Unit. She later returned to the
nursing home but was again taken to the hospital for difficulty in breathing on April 26tn
She was diagnosed with pneumonia and exacerbated COPD. They used nebulizer
treatments and Lasix to remove the fluid. She returned to the facility on May 6.
She is visited by Aging staff every 30 to 60 days. We also attend the quarterly care
plans where we meet with different staff ot the nursing home. The Home notifies us of
any changes in her condition.
It is the request of the Cumberland County Aging and Community Services that the
agency retain Guardianship of Person for Patricia Woolfson. The Golden Living Center-
Camp Hill has demonstrated more than adequate quality of care for her medical
problems. We believe, therefore that Ms. Woolfson should continue to reside in that
nursing facility.
ESTATE
At time of the Guardianship Ms. Woolfson had a bank account with Integrity
Bank.$17,500.00 was transferred from this account to a new Guardianship account at
F&M Bank. There was $500 left in the old account to keep it open until her direct
deposit can be changed to the new account. Many of these funds were used to pay her
medical bills from the hospitalization.
Ms. Woolfson was approved for Medical Assistance starting in February 2012. Aging
became her Social Security Representative Payee. Her income is currently $934 from
Social Security and $782.90 from a pension. Golden Living receives $1578.79 for room
and board. The amount varies slightly based on number of days in the month. Ms.
Woolfson receives the $45 for personal use as allowed by Medical Assistance. The
Cumberland County Aging and Community Services receives $100 a month in
guardianship fees as provided by the court order. Balance as of February 28, 2014 was
$4890.94.
