The undersigned,Martha Shay,hereby acknowledges and otherwise affirms the
following:
1. I am the niece of the alleged incapacitated person,Blanche L. Risser. I
acknowledge receipt of the Petition to adjudicate Blanche L. Risser as an incapacitated person
and to appoint a guardian. I further acknowledge having received notification that a hearing has
been scheduled before the Honorable Thomas A. Placey on March 24, 2014 at the Cumberland.
County Courthouse. I further understand that if I wish to contest Blanche L. Risser being found
incapacitated and Stephen L.Risser, Il,being named as her guardian, I must appear in court at
that time and present evidence to the contrary.
2. 1 further wish to affirm that I feel that my aunt,Blanche L. Risser is
incapacitated and that it is in her best interest that Steven L. Risser, II be appointed as her
