Report for Guardian of Person and Estate �� r' � , f ; � Cumberland County Aging and Community Services [,-� �' �; � �°. �� v c� . c7 On October 9, 2009 Mr. Pague was admitted to Chambersburg Hospita�bfoi' unco�oll� o blood pressure, ambulation difficulty and a brain bleed. From there he was admitt�ti to -'' Claremont Nursing and Rehab Center where his stay is expected to be long term. On November, 12, 2009 a Guardianship hearing was held before Judge Ebert. He appointed the Cumberland County Aging and Community Services as Mr. Pague's Permanent Plenary Guardian of Person and Estate. PERSON Mr. Pague has the following diagnoses: chronic dementia, accelerated hypertension, history of alcohol abuse, obstructive sleep apnea, hypokalemia, depression, arthritis, right frontal lobe hematoma/lacunar infarct, ambulation dysfunction, muscle weakness, cognitive disorder, delusions, hyperlipidemia, pain. The medications he is on are risperidone .5mg for behavior issues, tetracycline 500mg for rosacea, citalopram 20mg for depression, clonidine .2mg for hypertension, hydrochlorothiazide 25mg for hypertension, lisinopril 40mg, meloxicam 7.5mg for hypertension, diltiazem 240mg for hypertension, pravastatin 40mg for hypertension, hetoconazole 2% cream for seborrhea dermatitis. In addition to his regular medications, Jim is on two anti-psychotic medications, Depakote 125 mg finrice a day and Seroquel 75 mg in am and 50 mg in evening. He was placed on these meds because of inappropriate behaviors. He is also taking Celexa 20 mg for depression. In December 2012 he had a tooth extracted. In July two Agency staff took him to Bonanza to celebrate his 65th birthday. In August of 2013 he was found outside looking for his car and saying he had worked on his brother-in-law's car the entire afternoon. A wander guard was put back on him. In September 2013 he was given a new rollator type of walker which he likes very much. He received a month of physical therapy to help him use it correctly. He is currently medically stable and seems to be doing well. He attends at least one activity a day. There have been no behavioral issues and his wanderguard was taken off. Staff also attends the quarterly care plan meetings with nursing home staff from nursing, activities, social services, and the dietician. It is the request of the Cumberland County Office of Aging that the agency retain Guardianship of Person for James Pague. Claremont Nursing and Rehabilitation Center. has demonstrated an adequate quality of care for his medical problems. We believe, therefore, that James Pague should continue to reside in that nursing facility. ESTATE During 2013 Mr. Pague received $832 in social security. This is his only income. Payments are made to Claremont Nursing and Rehabilitation Center for $687.00 a month. Cumberland County Aging receives $100 a month for a guardianship fee which was approved in the court order. The balance in his M&T checking account as of 10/24/13 was $4120.52. Attached is a financial summary of James Pague's income and expenses.
