On August 22, 2011 Donald Harvey was admitted to the Holy Spirit B��a�ai�al H�alth� �J

At the time he had been living in his own home with a caregiver that n�t only too�ovecn �
the house by moving her family in but also was in control of his finances. He rece�'ved
minimal care and was financially exploited. He had previously attempted suicide on
November 20, 2010 by drinking antifreeze.
On November, 1, 2011 a temporary Guardianship hearing was held before Judge
Edward Guido. He appointed the Cumberland County Aging and Community Services
as Mr. Harvey's Temporary Guardian of Person and Estate. The Permanent
guardianship hearing was held on December 2, 2011 and the Agency was appointed.
PERSON
At the time of the guardianship, Mr. Harvey was a resident at the Keller House, a senior
apartment house. The Agency then transferred him to Stonehedge Personal Care
Home in Perry County. He was later hospitalized and had surgery for an aortic
aneurysm. Following the surgery he was admitted to Claremont Nursing and Rehab
Center where he still resides.
At the time of his admission, he was diagnosed with hypertension, coronary artery
disease, s/p coronary artery bypass, edema, hx stomach ulcers, ambulatory
dysfunction, arthritis, s/p spinal fusion, hx prostate cancer, high cholesterol, major
recurrent major depression, suicide ideation.
Donald's behavior is quite improved from when he was admitted in October 8, 2012. AT
that time he had be very aggressive. He has greatly calmed down since then and
seems to have adjusted to the nursing home. He has been placed on medication for
diabetes type 2. In March he experienced a Urinary Tract Infection and was taken to the
emergency room. He was treated with an IV antibiotic. Overall, his health seems to be
slowly deteriorating. He is sleeping more. He attended the Christmas party and seemed
to enjoy himself. Our agency delivered presents to him that were donated and wrapped
by volunteers from Home Instead Home Health.

Staff visits with Donald every 30-60 days. Staff also attends the quarteriy care plan
meetings with nursing home staff from nursing, activities, social services, and the
dietician.
It is the request of the Cumberland County Office of Aging that the agency retain
Guardianship of Person for ponald Harvey. The Claremont Nursing and Rehabilitation
Center has demonstrated an adequate quality of care for his medical problems. We
believe, therefore, that Donald Harvey should continue to reside in that nursing facility.
ESTATE
Donald receives 4 checks toward his income, Social Security which was $1367 during
2013, Central PA Teamsters Pension of$304, State Employee Retirement of$397.57
and CUNA Mutual $137.93 for a total of$2206.50.
Monthly expenses include his room and board at Claremont at $1850.40, Freedom Blue
Insurance of$171.40, guardianship fee of$100 to Cumberland County Aging. He also
has copays towards his medications and had various medical bills not covered by
Medicare or his insurance. Occasional payments are still being paid to the Keller
Apartments. During 2013 they were paid $800.
In May the Agency made a pre-paid cremation arrangements with Hollinger's funeral
home. $1200 was paid towards it with a balance due of$810.
The process of having his house fore-closed upon is still ongoing.
Attached is a financial summary of Donald Harvey income and expenses. Also attached
is a spread sheet representing his checking account register.
