Cook, and not in their individual capacities ("Owner"), having a last known address as the property below, and all that certain tract of land including improvements thereon, owned by Owner and described as follows: 4 Thornhill Court Carlisle, PA 17015 Tax Property Map: 40-24-0748 Parcel No.: 086 for unpaid water and/or sewer services rendered from October 1, 2011 to December 31, 2011. Said services were rendered and charged pursuant to applicable rate resolutions of the South Middleton Township Municipal Authority.
