My wife, Linda E. Boughter, desires to submit a claim against the "Estate of Ruth M.Boughtee'for healthcare
services rendered from June 2004 to May 7, 2013.
Linda's responsibilities as a primary caregiver and companion were numerous. It changed as the condition of
Ruth's health deteriorated. Linda provided dedicated around the clock care (240).
There was much to be done.
The following is a list of what her duties consisted of;
1. Assist with personal care: To include,
a. Dressing,Bathing,Linda would change and bathe Ruth multiple.times daily due to
incontinence.
b. Toileting.
c. Meals: To include.
Linda prepared all Ruth's meals.
She served the meals.
Linda provided assistance feeding Ruth.
Linda cleaned up after Ruth's meal(s).
D. Laundry:
a. Wash personal clothing,belongings, bath linens, etc.1,61-.u•"-9)
• Note: How often she washed per day depended on how many times Ruth was incontinent
throughout the course of a day.
2. Provide companionship, emotional support and entertainment
3. Manage Health Care Needs:
a. Monitored medicine(s), called in prescriptions, picked up prescriptions
b. Provided skilled care
c. Communicated with healthcare providers
d. Coordinate all medical appointments
e. Advocated for quality healthcare when needed.
4. Provide around the clock supervision
(Because Ruth had progressive Dementia)
a. To prevent injuries.
b. To prevent frequent falls.
c. To curb wandering.
5. Linda maintained a suitable, healthy supportive household
a. Grocery shopped for Ruth's special needs.
b. Cleaned.
c. Cooked and prepared special dietary meals
d. Laundry service for Ruth.
e. Other duties, involving Ruth's special needs.
6. Linda provided Transportation
a. Drive Ruth so she could complete errands
b. See Ruth made her medical appointments
c. Transport Ruth for special treatments
d. Provide for Ruth's recreational needs
7. Take care of all of Ruth's finances
(Due to cognitive challenges i.e. "Progressive Dementia")
a. Pay all Ruth's bills, and manage financial effects_
This list is not all inclusive.
The prevailing rate for a skilled, live-in caregiver in 2011 was $170.00 er da ! Linda is
requesting $400.00 a month for 8 years, 11 months and 7 days.
$400.00 x 12 =$4,800.00 x 8 years=$38,400.00
400.00 x t t months= $4,400.00
7 days=$100.00
Total requested: $42,900.00

Ron and Lew,
Because this claim is in the interest of an individual, not in the interest of the
estate as a whole, and we represent the estate, Linda Boughter will need to file
this claim with the Register of Wills office in Cumberland County, or get separate
legal counsel regarding this matter. The claim must be filed before August 14,
2014, which is one year from the date that the letters were advertised.
