PETITION FOR CHANGE OF NAME
TO THE HONORABLE, THE JUDGES OF SAID COURT:
AND NOW, comes the Petitioner, Caitlin Marie Krawchuck, by and through his counsel
Derek Cordier, Esquire, and files this Petition for Change of Name and in support hereof states the
following: ,
1. Petitioner, Caitlin Marie Krawchuck wishes to have his name changed because he no longer
wishes to be known as Caitlin Marie Krawchuck and would rather be known as Caden James
Krawchuk, as he is transgendered and wishes to be so identified.
2. The Petitioner, Caitlin Marie Krawchuck, is an adult over the age of twenty-one years, and
currently resides at: 1116 Grandia Flora Drive, Mechanicsburg, Cumberland County,
Pennsylvania 17112.
3. During the five years preceding the filing of this Petition, the Petitioner has resided in the
following places:
From 2000,to the present: 1116 Grandia Flora Drive, Mechanicsburg, Cumberland County,
Pennsylvania 17112.

4. There are no judgments or decrees pending against the Petitioner in this or any other
jurisdiction.
5. That the proposed change in the Petitioner's name, if granted, will not be detrimental to the
interests of any other person and will not be against the public interest.
6. This Petition is not being filed to defraud creditors.
7. Petitioner is not married.
WHEREFORE, the Petitioner respectfully requests that this Court, pursuant to the Act
of December 16, 1982, P.L. 1309,No. 295, § 2, 54 Pa. Cons. Stat. Ann. § 702, order the
following:
a. to fix a hearing on this Petition;
b. to direct that notice be given on this Petition and hearing; and
c. to enter a decree changing the Petitioner's name from Caitlin Marie Krawchuck to:
