That the claimant is EFCO CORPORATION which was engaged in business as a sub-subcontractor of HERS HOCKS INC, POBox 5800, Harrisburg, PA 17105 and provided custom storefront, curtainwall, windows and/or doors together with the installation thereof at the hereinafter described property owned by CARLISLE HOSPITAL & HEALTH SERVICES (owner[ s]), to furnish materials and/or labor for the improvement / construction to the real property as stated above, at the request of HERSHOCKS INC (subcontractor), defendants herein. 2. That the owner[s], or reputed owner[s] of the building known as: Carlisle Hospital, 246 Parker Street, City of Carlisle, County of Cumberland, State of Pennsylvania, Parcel #21-0322-0231-0000000-04 Book 117 Page 287, is(are) CARLISLE HOSPITAL & HEALTH SERVICES, Attn: Facilities, 246 Parker Street Carlisle, P A 17013. 3. That the nature and kind of work done was the furnishing of materials and/or labor in connection with the erection, construction, restoration, or repair and improvement of the aforesaid property. 4. The amount unpaid and owning to the claimant, EFCO CORPORATION, by the above- captioned owner[ s] or reputed owner[ s] for the aforesaid material and/or labor is in the sum of $122,497.38. 645961 ...
