C.S. §701 et seq., avers as follows: 1. Petitioner is an adult individual residing at United Methodist Home for Children located at 5120 Simpson Ferry Drive, Mechanicsburg, Cumberland County, Pennsylvania 17050. 2. Petitioner was born on September 20, 1995, and is 18 years old. 3. Petitioner is now desirous of changing her name to Shyanne Keila Becker. 4. Prior to residing at 5120 Simpson Ferry Drive, Mechanicsburg, Pennsylvania, Petitioner has resided in the following locations over the last five years; (a) Sarah A. Reed Children's Center in Erie, Pennsylvania; (b) a foster home in Lititz, Pennsylvania; (c) 152 East Chestnut Street, Lancaster, Lancaster County, Pennsylvania; (d) 509 Lafayette Street, Lancaster, Lancaster County, Pennsylvania; (e) 330 Mill Street, Lancaster, Lancaster County, Pennsylvania. 5. Petitioner is currently in the custody of the Lancaster County Children and Youth Services Office and attends school at Cedar Cliff High School where she has recently completed her junior year. r(0 3 9S��a Y 4f 40 ?SIP ;y 6. Petitioner's mother's maiden name was Becker, and while Petitioner's mother has predeceased her, Petitioner desires to change her name to her mother's maiden name. Petitioner's father, Sean Fake,is wanted for rape of a child,and Petitioner has had no contact with him since she was 9 years old. Accordingly,it is in Petitioner's best interest to take her mother's maiden name of Becker. 7. Petitioner has no criminal record. 8. There are no judgments or decrees of like character of record against the Petitioner as indicated on the attached Certification. WHEREFORE, Petitioner respectfully requests that this Honorable Court: (a) Fix a hearing on this Petition; (b) Direct that notice be given of this Petition and hearing; and (c) Enter a decree changing Petitioner's name from Shyanne Keila Fake to Shyanne Keila Becker.
