2. Three (3) sets of additional signature pages for the Rev-1500 Inheritance Tax Return to be
clocked-in in your office and returned to my office.
3. A check in the amount of$206.70 which represents the additional tax due on the return.
4. Three (3) original copies of the Inventory to be clocked-in in your office, one of which is to be
returned to my office.
5. A self-addressed, stamped envelope for return of the clocked-in signature pages for the Rev-1500
Inheritance Tax Return and Inventory.
