PERSON AND ESTATE OF : KENNETH CAMPBELL FINE : ANNUAL REPORT OF THE PLENARY GUARDIAN5 OF THE PERSON AND ESTATE OF KENNETH CAMPBELL FINE David R. Fine and Beth Campbell Fine ("Guardians") hereby offer the annual report required by the Court's guardianship order of August 15, 2012. a. the current address and type of placement of Kenneth Campbell Fine Kenneth continues to live with his parents (the Guardians) at 344 North 25th Street, Camp Hill, Pennsylvania. b. any major medical or mental problems experienced by Kenneth Campbell Fine Kenneth has had no major medical or mental problems in the year preceding the filing of this report. He had a dental procedure in which three wisdom teeth were removed and one other tooth was removed because there was an abscess in the gum above it. c. a brief description of Kenneth Campbell Fine's living arrangements and the social, medical, psychological and other support services he is receiving Kenneth lives with his parents in Camp Hill, Pennsylvania. He attends The Vista School in Hershey, Pennsylvania. Kenneth receives behavioral-health services from The Vista Foundation. Kenneth receives support-coordination services from Cumberland/Perry MH/IDD. In the past year, Kenneth's supports team has focused � particularly on his transition to adult services. Recently, Kenneth's IEP team agreed to send Kenneth to the Hanover Street Neighbors program in Carlisle, Pennsylvania, one day a week to see if it would be an appropriate placement for Kenneth when he graduates from the Vista School. (Neighbors is a day program for adults with developmental or cognitive disabilities.) d. the opinion of the plenary guardians regarding whether the guardianship should continue, be terminated or be modified and the reason for that opinion The Guardians believe the guardianship should be continued according to its present terms. e. if Kenneth Campbell Fine is living separately from the plenary guardians, the number of times the plenary guardians visited Kenneth Campbell Fine in the preceding year Kenneth is living with the Guardians. f. the full value of Kenneth Campbell Fine's financial estate Kenneth has no financial estate. g. all income received by Kenneth Campbell Fine in the previous year Between August 1, 2013, and July 31, 2014, Kenneth will have received a total of $6,037.75 in Supplemental Security Income and related payments from the Department of Public Welfare of the Commonwealth of Pennsylvania. h. all expenditures made from Kenneth Campbell Fine's financial estate and income $6,000 of the income described in (g) has been applied to the mortgage on the home in which Kenneth lives with the Guardians. The remainder of the income remains in Kenneth's bank account. 2 . . i. all expenditures made on Kenneth Campbell Fine's behalf by the plenary guardians from their own funds. With the exception of the payment noted under (h), above, the Guardians have paid fully for Kenneth's lodging, food, clothing, entertainment, fitness training and vacations. These expenditures include, but are not limited to, twice-weekly sessions with a personal trainer, membership at local pools, attendance at musical shows, a week-long trip to the Outer Banks in North Carolina, several trips to the beach in Maryland, a vacation in the Bahamas and a vacation in Ft. Lauderdale, Florida. The Guardians (or their private health insurer) have paid for all medicaUdental expenses for Kenneth in the previous year that were not covered by private insurance or Medical Assistance (Medicaid).
