PETITION FOR CHANGE OF NAME
AND NOW, comes Petitioner, Tracy Besselman, through her undersigned counsel,
request that this Honorable Court order a change of name of her daughter, Eilene Meghan
Dechene, pursuant to 54 Pa.C.S. § 701 et seq., and in support thereof avers as follows:
1. Petitioner, an adult individual, resides at 402 Lake Drive, Mechanicsburg,
Cumberland County, Pennsylvania.
2. Petitioner is the natural mother of the minor child, Eilene Meghan Dechene
(hereinafter"Minor Child").
3. Petitioner is currently married to David Besselman since 2005.
4. Petitioner brings this Petition on behalf of the minor child who seeks a name
change to Eilene Meghan Besselman. G ,� , l

5. The Minor Child was born on August 28, 2000 and is currently 13 years old.
6. In the last five years, the Minor Child has lived with the Petitioner at 402 Lake
Drive, Mechanicsburg, Pennsylvania.
7. The Minor Child's father, Patrick Dechene, is deceased, having died on
August 4, 2013.
8. The Minor Child, through Petitioner, requests the name change for the
following reasons:
(a) Minor Child's natural father is deceased.
(b) Minor Child has lived with Petitioner and Petitioner's husband/Minor
Child's stepfather for the last 10 years. Both Petitioner and Petitioner's husband have the
last name of"Besselman."
(c) Minor Child lives also with her half brothers, Gavin (d.o.b. March 26,
2008) and Connor (d.o.b. June 26, 2009). Minor Child's step siblings all have the last
name of"Besselman."
(d) The Minor Child's relationship with her natural father was harmful to the
Minor Child due to abuse, and the Minor Child does not want to continue to have father's
last name.
(e) Therefore, it is in the Minor Child's best interest to change her last name
to Besselman so as to have the same last name as her mother, step-father and siblings and
to continue the healing process by establishing a fresh start for the Minor Child.

9. The proposed change in the Minor Child's name, if granted will not be
detrimental to the interests of any other person and is not against public interest.
10. The Petition is not filed to defraud any creditors.
WHEREFORE, Petitioner on Behalf of Minor Child requests the Court, pursuant
to'54 Pa.C.S. § 701, for the following:
a. to fix a hearing on this Petition;
b. to direct that notice be given on this Petition and hearing; and
c. to enter a decree changing the Minor Child's current name from Eilene
Meghan Dechene to Eilene Meghan Besselman.

AND NOW, comes Petitioner, Tracy Besselman, by and through her undersigned
counsel, and request this Honorable Court continue the hearing scheduled for November 6, 2014,
regarding the Petition for a Name Change in this matter and, in support thereof, avers as follows:
1. On August 7, 2014, Petitioner filed a Petition for Name Change.
2. On September 26, 2014, a hearing on the Petition for name change was scheduled

3 Unfortunately, Petitioner's Counsel inadvertently failed to publish the Notice of
Hearing within seven (7) days of the date of the hearing as required by the Court Order of
September 26, 2014.
4. Therefore, in order to timely publish notice of the hearing, Petitioner requests a
short continuance of the scheduled hearing.
WHEREFORE, Petitioner's Counsel respectfully requests that this Honorable Court
continue the hearing in this matter to the soonest available date.
