Petitioner has been a resident of Cumberland County,Pennsylvania for at least six (6)months previous to the filing of this Petition. 4. Petitioner desires to change his name from Jerrell Thomas Rogers to Jerrell Thomas Felton. Law Offices of 5. Petitioner is seeking a change of name for the following reasons: Saidis Sullivan a. Petitioner's father, John Jereith Rogers,has had no relationship with & Rogers Petitioner. 26 West High Street Carlisle,PA 17013 b. Petitioner would like to have the same surname as his mother. C. Petitioner does not wish to change his name for any illegal purpose,to avoid the payment of taxes or other debts, or to avoid any obligation. Petitioner has been fingerprinted and his fingerprint card is attached hereto as Exhibit "A" and by reference incorporated herein. 7. Petitioner's counsel has searched the index in the Prothonotary's Office in the Court of Common Pleas of Cumberland County and there are no judgments or decrees of like character of record against Jerrell Thomas Rogers.
