PETITION OF JANET ESTHER JONES CHANGE OF
NAME TO JAN JONES
1. Your Petitioner is Janet Esther Jones, an adult individual, who resides at 41
Sutherland Way, Mechanicsburg, Cumberland County, Pennsylvania.
2. Petitioner was born on February 14, 1954. A certified copy of Petitioner's birth
certificate is attached hereto, marked Exhibit "A" and incorporated herein by reference.
3. Petitioner's parents are deceased.
4. Petitioner desires that, by order of this Court, Petitioner's name be changed to Jan
Jones.
5. Petitioner desires the name change described above because of her recent
marriage and some records still show her former name of Esposito; furthermore, she has
been known as 'Jan Jones' since her marriage and most persons address her as 'Jan.'
6. Petitioner was married on May 9, 2014; Petitioner's husband does not object to the

proposed name change.
7. Petitioner does not have a criminal record; we are submitted a fingerprint
contemporaneously with this Petition and we anticipate the standard criminal record check
to confirm this assertion.
8. There are no judgments or decrees of like character of record against the
Petitioner.
9. Your Petitioner does not know of anyone who objects or, upon notice would object,
to the name change proposed herein.
WHEREFORE, Petitioner requests this Honorable Court to set a hearing to consider
the requested change of name as contained herein, to ultimately enter an Order changing
Petitioner's name as requested herein, and to provide any other relief that this Court
deems appropriate.
