TO THE HONORABLE, THE JUDGES OF SAID COURT:
The Petition of Kristina Leigh Kovach, Petitioner, respectfully represents:
1. Petitioner, Kristina Leigh Kovach, is seeking a change of name to Kristina
Kovach Montgomery for the following reasons:
a. Petitioner was married to Donald Andrews Montgomery on July 4, 2014
in Cumberland County, Pa.
b. Petitioner became employed in family business Riverside Hearing
Services d/b/a as Beltone Hearing, 5405 Jonestown Road, Harrisburg
Pa., 17112, in June 2012 and intends to continue employment
with said business for foreseeable future
c. Petitioner's maiden name (Kovach) has been associated with
business activities and registrations since 1996
d. Petitioner desires to maintain maiden name Kovach on all business-
related documents and professional licenses
2. Petitioner currently resides at 719 MacArthur Drive, Carlisle, PA 17013 and
has resided at the following address(es) in the past five years:

3. There are no judgments or decrees of like character of record against
Petitioner.
4. Petitioner has no criminal record.
5. Petitioner does not seek a change in her name to avoid creditors, debts, or
other obligations and responsibilities.
WHEREFORE, Petitioner respectfully requests this Court, pursuant to the Act of
December 16, 1982, P.L. 1309, No. 295, Section 2, 54 Pa. Cons. Stat. Section 702, for
the following:
a. To fix a hearing on this Petition;
b. To direct that notice be given of this Petition and hearing; and,
c. To enter a decree changing the Petitioner's name from Kristina Leigh Kovach to
Kristina Kovach Montgomery.

AND NOW, this 8TH day of SEPTEMBER, 2014, the Petition for Change of
Name filed by Petitioner Kristina Leigh Kovach, was filed in the above named Court,
praying for a decree to change her name to Kristina Kovach Montgomery.
A hearing is scheduled for FRIDAY, OCTOBER 17, 2014, at 9:30 a.m. in
Courtroom # 3, Cumberland County Courthouse, Carlisle, Pennsylvania, as the time and
place for the hearing of said Petition, when and where all persons interested may appear
