C.S. 5111(a), AND FOR DEPOSIT OF THE FUNDS INTO A RESTRICTED BANK ACCOUNT TO THE HONORABLE, THE JUDGES OF SAID COURT: The Petition of SHERRY L.ANDERSON and CAROLJ.SNYDER,respectfully represent. that I. The minor is EVAN C.ANDERSON, born September 20, 2000,who resides part-time with his mother, Sherry L. Anderson, and also with his father, Donald E. Anderson,jr. 2. The Petitioner SHERRY L.ANDERSON,biological mother of the minor child, resides at 914 Bosler Avenue, Lemoyne, Pcnnsyhvatlia 17043. 3. The Petitioner CAROLJ.SNYDER,maternal grandmother of the minor,resides a(3851 Dora Drive, Harrisburg, Pennsylvania 17110. 4.. The biological father of EVAN C. ANDERSON, is DONALD E. ANDERSON, who resides at 558 Walnut.Street, Lcmoync,Pennsylvania 17043,who consents to the Petition as being presented, which Consent is attached hereto. 5. No guardian has hcen appointed, and it is necessary that a guardian of the estate of the minor be appointed because the minor is named as a Bene1iciar}r�in the Will of his h cat- aunt, Sara J. Ross (67-13-0145), and receives a distribution from his great-aunt's estate equal to$71,714.48(see BE LCO Community Credit.Union statement attached as Exhibit "A"). 6. SARAJ. ROSS ("Decedent") departed this Earthly life on December 28, 2012, leaving a Last Will and Testament dated August 15, 2008,which was probated in the Olrcc of the Register of\Alills of York County, Pennsylvania on January 24, 2013. LAW 0117CEOF 7. Letters Testamentary were granted to your Petitioner, Sherry L. Anderson, as evidenced Wm.D.S©imcK III by the information contained in Register of Wills File No. 67-13-0145. 8. The interest of Ewin C. Anderson should be represented by Sliary L. Anderson and Carol J.Snyder,your Petitioners in this matter,because the Testatrix specifically provided that her niece, Sherry L. Anderson, be appointed Truslce of the sums reccivcd by the Bencliciary at the lime of her death. 9. Control of the account of' Evan C. Anderson shall be maintained until September 20, 2018, the dale upon which Evan C. Anderson attains the age of 18 years. WHEREFORE, Petitioners request that the Court appoint SHERRY L. ANDERSON and CAROLJ.SNYDER as Guardians of the Estate of Evan C.Anderson,and authorize the distribution of assets from the Estate of Sara J. Ross, Deceased, to hcr, for the benefit of the minor, Willi which proceeds to purchase,an insured savings certificate in her name, as Guardians of Qtc Estate of the ahove-nauned minor, marked as set forth in die Decree attached hereto.
