SMALL ESTATE PETITION
The Petition of Good News Consulting, Inc., by and through her counsel, Amanda Snoke
Dubbs, Esq.,respectfully states that:
1. Orville C. Zeiders died on July 23, 2014, a patient at Manor Care Carlisle, 940 Walnut
Bottom Road, Carlisle, Pennsylvania, 17015.
2. Petitioner is of the following intestate heirs of the Decedent.
a. Lois Sorrano
403 Venice Avenue
Carlisle, PA 17013
3. Decedent died intestate and no Letters of Administration have been issued.
4. Petitioner is Good News Consulting, Inc. who was the decedent's court appointed
guardian since December 14, 2011.
5. The assets of Decedent total less than$50,000.00 and are as follows:
a. Check from HCR Manor Care $1,031.25
b. Check from HCR Manor Care $2,186.23
c. Check from Heartland Healthcare Service $ 71.99
d. Balance in M&T Checking Account $ 148.97
Total $3,438.44
6. The following creditors, who are all of the creditors of the estate of whom Petitioner has
knowledge, are unpaid:
a. Department of Public Welfare $155,762.99
Total $155,762.99
7. A Pennsylvania Transfer Inheritance Tax Return has been filed with the Register of
Wills, and there was no inheritance tax due.
8. Decedent was not a fiduciary of another estate and was not a surety on the bond of a
fiduciary.
9. All of the above-named parties in interest will be served by U.S. First Class, Certified
Mail and Affidavit of Service will be filed with the Court.
Wherefore, Petitioner respectfully requests that this Court award Decedent's estate as
follows:
1. Cost of Administration
a. Amanda Snoke Dubbs, Esq—Counsel Fee $400.00
in connection with the settlement of the estate
b. Good News Consulting, Inc. —Administration $400.00
in connection with the settlement of the estate
c. Filing fees—Court of Orphan's Court $ 70.50
(to be paid to Amanda Snoke Dubbs)
2. Costs of Funeral and burial and medical care within six months of Decedent's death
a. Department of Public Welfare $2,567.94
3. Balance $ 0.00
Total $3,438.44
