AND NOW,comes Monroe Township Municipal Authority, of 1220 Boiling Springs
Road, Mechanicsburg, Pennsylvania, by and through its Solicitor, Michael J. Pykosh, Esquire,
and files the following municipal claim against Tammy E. Hollinger and David Cochran, owning
property located at 1380 Zimmerman Road, Carlisle, PA 17015. Tammy E. Hollinger and David
Cochran are the owners of all that certain lot of ground, including any improvements thereon,
being more fully described as follows:
ALL THAT CERTAIN piece or parcel of ground situate in the Township
of Monroe, County of Cumberland, and State of Pennsylvania, more particularly
bounded and described as follows,to wit:
BEGINNING at a point marked by a nail at the intersection of the center
line of Shuman Drive (T-551)with the center line of Zimmerman Road(T-5533);
thence by the center line of Zimmerman Road,North 79 degrees, 12 minutes, 10
seconds West, 471.68 feet to a point; thence by other lands of the Grantors,North
10 degrees 47 minutes 50 seconds East, 304.45 feet to a point; thence by the
same, South 79 degrees 12 minutes 10 seconds East, 386.78 feet to a point in the
center line of Shuman Drive; thence by the center line of Shuman Drive, South 04
degrees 47 minutes 10 seconds East, 316.07 feet to a point,the place of
BEGINNING.
BEING THE SAME PREMISES which Tammy E. Hollinger, by Deed
dated November 18, 2004 and recorded January 10, 2005 at the Recorder of
Deeds Office in Cumberland County, Pennsylvania, in Book 267, Page 440,
granted and conveyed unto Tammy E. Hollinger and David Cochran, both

Monroe Township Municipal Authority hereby files this Municipal Lien for Services and
Sewer Rates levied and assessed from through and including. Said Authority services and sewer
rates were levied and assessed pursuant to Authority Resolution No. 02-04, same being known as
a Resolution of the Board of Monroe Township Municipality Authority imposing user charges to
be collected from the owner of each improved property served or to be served by the Authority's
sewer system, in conjunction with all prior relevant Resolutions and Ordinances, including,but
not necessarily limited to, Monroe Township Ordinance No. 3 of 1978 and Monroe Township
Ordinance No. 99-6, as amended; and providing for payment and collection of such user charges.
Attached hereto and incorporated herein are the statements forwarded by Monroe Township
Municipal Authority to for sewerage due to be treated from October 1, 2013 through December
31, 2013,which services total is in the amount of seven hundred seventy and 00/100 ($770.00)
dollars, which sum was duly assessed against, and the real property described herein from the
date of said billing statements as attached hereto, and for which sum, with interest in the amount
of ten(10%)percent, along with penalties and all costs, including attorneys' fees, a municipal
lien is claimed against the above-described real property and premises in accordance with the
Act of May 16, 1923, P.L. 207, §7, as amended, 53 P.S. §7101, et seq, as amended(Municipal
Claims and Tax Liens).

Monroe Township is contacting you in regards to the Delinquent Sewer Account Agreement that
was signed on December 4,2013 by David Cochran,for real estate owned by you,same being known
and numbered as 1380 Zimmerman Road,Carlisle,PA,and Account#1032.E
The Agreementstates thatylou are res onsibie to pay a minimum of L100,00 per month o0.
the arrears and also keep up with the amount billed.for the current quarter.
The last payment we received from you was on December 5,2013 in the amount of$100.00.
You currently have an outstanding balance of$770.00.
According to our records you have not kept up with the Agreement terms. Therefore,within
five(5)days from the receipt of this notification you are required to resume the payment
arrangements stated in the Payment Agreement,or the Authority will resume the lien process against
