C, ; ry m Q C O O T7 PERSON o� : n On July 8, 2011 a guardianship hearing was held before Judge Guido. Cuml;erland�, y o County Aging and Community Services was appointed his Permanent Plenary Guaftlian -:l of Person and Estate. Mr. Benami had lived at the Molly Pitcher Hotel and moved to public housing when it closed. At the time of the hearing, Mr. Benami was a patient at a Holy Spirit Behavioral Health. Upon discharge, he was placed at Manor Care-Carlisle where he still resides. He was diagnosed with: dementia, hypertension, anemia, congestive heart failure, generalized weakness, unsteady gait, legally blind, glaucoma, chronic paranoid schizophrenia, anxiety, history of rambling speech and history of disorganized speech. He is at risk for falls due to his unsteady gait and vision impairment. He has episodes of incontinence of bowel and bladder. His medications must be administered to him and he requires cueing and reminders for activities of daily living, such as personal care. He requires total assistance with his instrumental activities of daily living such as laundry and meal preparation. Mr. Benami seemed to adjust well to life at the nursing home. He eats his meals in the dining room, likes the food, and has no issues with his roommates or other residents. He spends a lot of time in his room listening to his radio which is his favorite activity. A talking watch was purchased for him from the Tri-County Blind Association. Funeral plans were mace with Hollinger funeral home. The plan is for cremation with the remains being returned to the family as requested by his brother Alfred Means. In July of this year David began to experience a decline. He has been having trouble swallowing and has been placed on a mechanical soft diet. He is also retaining fluid. The home is continuing to do lab work to watch his potassium level. Mr. Benami is visited by Aging staff every 30-60 days and staff attend quarterly care plans where they meet with nursing, dietary, activities, and social services staff. His brother, Alfred Means who lives in Philadelphia is kept informed of our visits. Manor Care-Carlisle has demonstrated an adequate quality of care for his medical problems. We believe, therefore, that David Benami should continue to reside in that nursing facility. ESTATE After becoming guardian, the agency set up a guardianship checking account for him at F&M Trust. Application was made to Social Security for the agency to become rep payee and this was granted. His income includes Social Security which currently is $486 and $90 from the VA. The monthly payment to Manor Care is currently $341.05. Cumberland County Aging receives a monthly guardianship fee of$100 as allowed by Medical Assistance and was included in the court order. The only other expense this year was $150 for new clothes. These were necessary due to his loss of weight. A summary of David Benami's income and expenses are attached.
