I, Anna Borro Hays, certify that I served a true and correct copy of the
Petition, Citation and Preliminary Order of Court and Notice as required by 20 Pa.
C.S. § 5511(a) upon Martha Jean McCurdy, the alleged incapacitated person, by
personal service and explanation in language and a manner which Martha Jean
McCurdy would reasonably under on September 25, 2014 at 11:00 a.m. at her
residence at 11 West Dulles Drive, Camp Hill, Pennsylvania 17011.
I make this affidavit subject to the penalties of 18 Pa. C.S. §4904 relating to
unsworn falsification to authorities.
