Birth Date and Place: The minor child, currently named Randy Allen Divelbiss, Jr., was born on April 4, 2009, in Cumberland County, Pennsylvania. A certified copy of the minor's birth certificate is attached to this Petition. 2. Birth Name: The minor was named Allen Divelbiss, Jr., at birth, and as of the date of this Petition, the minor has always been known by that name, with the following exceptions, if any: N/A 3. Residence: The minor resides at 172 Virginia Avenue, Carlisle, Pennsylvania, 17013, and has lived at this address since August 2014. 4. Minor's Parents: The mother of the minor child is Heather A. Holley, Petitioner, and she resides at 172 Virginia Avenue, Carlisle, Pennsylvania, 17013. The father of the minor is Randy Allen Divelbiss, Sr. 5. Reason for Name Change: Petitioner seeks to change said minor's name to Randy Allen Holley, and offers the following reason for seeking to do so: The minor child currently shares the last name of Father, who has not been involved in 4I157S J''f' P e/0-7? p;if-3 l239 -3 f1723 the minor child's life since birth. Father has only seen the minor child a handful of times since the child's first birthday, and does not maintain contact with the child. The minor child does not recognize Father's last name, and believes his last name is Holley. 6. Legal and Creditor Status: The minor does not have any outstanding judgments, and has never been convicted of a crime. 7. Additional Declarations: Father has routinely failed to pay any form of support for the minor child, and willfully ignores all domestic relations proceedings and contempt hearings for arrearages. WHEREFORE, Petitioner respectfully requests that this Honorable Court order the minor's name changed as follows: Randy Allen Divelbiss, to Randy Allen Holley.
