PENNSYLVANIA JOHN SAILON, NO. 2014 - 5�s3 MLD Owner MUNICIPAL CLAIM Township of Hampden ("Claimant") files this Municipal Claim against John Sailon, having a last known address of 4060 Sara Drive, Apartment 204, Uniontown, OH 44685 ("Owner") and all that certain tract of land including improvements thereon, owned by the Owner and described as follows: 5205 Wertzville Road Enola, PA 17025 Tax Property Map: 10-14-0840 Parcel No.: 003 for unpaid sewer and/or trash services rendered from February 1, 2014 through July 31, 2014. Said services were rendered and charged pursuant to Hampden Township Code of Ordinances Chapter 18, Part 2 and/or Chapter 20, Part 1, as amended. LAW OFFICES Attached hereto is a statement of the kind and character of the services so SNELBAKER& BRENNEMAN, P.C. 037/6 311 r7o rendered and the prices charged therefor, said statement being a true and correct copy of the Owner's account with the Claimant, which sum of$595.34 was duly assessed against the above Owner and premises beginning February 1, 2014 and for which sum, which includes interest, costs and penalties,together with attorney's fees pursuant to Section 209, Part 2, Chapter 18 of the Township of Hampden Code of Ordinances, a lien is claimed against the above premises in accordance with the Municipality Authorities Act of June 19, 2001, P.L. 287, 53 P. S. § 5601, et seq-, as supplemented and amended and the Municipal Claims and Tax Lien Act of May 16, 1923, P.L. 207, 53 P.S. § 7101, et seq., as supplemented and amended.
