Elscheid, the mother of Makayla M. Elscheid-Murlin, by and through her attorney Andrew C. Sheely, Esquire, petitions the court for the change of her daughter's name in accordance with 54 Pa. C.S.A. 701, et seq, based upon the following: 1. Petitioner is Jessica L. Elscheid, an adult individual who resides at 511 Second Street, New Cumberland, Cumberland County,, Pennsylvania. 2. Petitioner is the natural mother of the minor child, Makayla M. Elscheid-Murlin, now age nine (9) years, having been born on October 2, 2005. Petitioner files this Petition on behalf of her said daughter. 3. Petitioner and Makayla M. Elscheid-Murlin have resided:: at the 511 Second Street, New Cumberland, Cumberland County, Pennsylvania address for two (2) continuous years prior to the date of filing this Petition. tts-.75--ed kit C#' v3o9 e 7P-3/2ooS 4. Petitioner asks this court to change the name of her daughter from Makayla M. Elscheid-Murlin to Makayla M. Elscheid. 5. Petitioner wishes to have this court change the name of her daughter to Makayla M. Elscheid so that the child's name will be the same of her mother, the Petitioner. 6. The father of the child, James J. Murlin, Jr., died September 24, 2012 and therefore no notice is required to be given to him and no other person has any legal or custodial rights of the minor child. 7. Petitioner wishes to have this court change the name of the minor child to minimize family and social trauma related to certain criminal charges committed by James J. Murlin, Jr. wherein Petitioner and Makayla M. Elscheid-Murlin were victims of wrongful conduct committed by James J. Murlin, Jr. 8. The child, Makayla M. Elscheid-Murlin has no criminal record and there are no judgments or other adverse decrees entered against her. 9. Petitioner does not seek a change in the name of Makayla M. Elscheid-Murlin to avoid creditors, to avoid debts, or to avoid other obligations or responsibilities. 10. No fingerprinting is required in this action due to the age of the minor child, Makayla M. Elscheid-Murlin. 2 WHEREFORE, Petitioner, Jessica L. Elscheid, respectfully requests this Court to change the legal name of her daughter from "Makayla M. Elscheid-Murlin" to "Makayla M. Elscheid".
