PETITION FOR A NAME CHANGE OF A MINOR CHILD
1, Bryanna S. Smith, Petition to change my chiid's name, Bai|eigh S. Bevins to Baileigh S. Smith, and as
grounds therefore respectfully represents as follows.
1. That the name of Baileigh's father is Jonathan C. Bevins, and 1, Bryanna S. Smith, am
BaiIeigh's mother. My maiden name is Bryanna 5. Brooks.
2. Baileigh was born on the 23 day of July, 2008, in the city and state of Camphill Pennsylania
at Holy Spirit Hospital.
3. Baileigh resides at 2735 Ritner Highway CarIisie PA, 17015.
4. Baileigh has rio felony conviction record.
5. Baiieigh is not .presently incarcerated or on probation in any court.
6. Baileigh is not required to be registered with the Sex Offender and Crimes against Minors
Registry.
7. Baileigh has not previously chariged her name before.
8. 1, Bryanna S. Smith, do not ask to change her name for any illegal or fraudulent purpose, or
to infringe upon the rights of others, but because I wish her last name to be that as the
same as mine. I have primary physical and legal custody of the child as of July 21, 2014 after
having him investigated by Cumberland County Children and Youth and Carlisle Borough
Detectives. Jonathan has not been consistently involved with the child from birth. There has
been no visitation between Jonathan and Baileigh since May 25, 2014 on Jonathan's terms.
Jonathan has a bad reputation for numerous reasons. Such as his criminal record and
criminaI behavior. Jonathan continues to make rio effort to see or speak to the child.
Jonathan has failed to make any support payments as granted to me by the Court of
Common Pleas of Cumberland County, Domestic Relations Section. Our support order is
PACSES Case Number 30711OS9Sand began onJuly S,2O14.OnAugust 21,2O14|was
informed that Jonathan had reiocated to the state of Florida. By relocating without notifying
me or the courts, he is in contempt of our custody agreement as well as our child support
order. I have contacted Jonathan numerous times asking for his new address and he will not
give it to me. I have asked his mother for his address as well she will not give it to me. The
last time I tried to contact Jonathan his phone was no longer in service. All of these events
lead me to ask the courts to change my child, Baileigh's last name to that of mine. Jonathan
is not involved and shows no interest in being involved.

1. A hearing shall be held on said Petition on the 24th day of November, 2014,
at 11:00 a.m. in Courtroom Number 1, Cumberland County Courthouse,
Carlisle, Pennsylvania.
2. The Petitioner shall publish notice of this hearing in two newspapers of
general circulation, one of which may be the official paper for the publication
of legal notices in this county. Personal notice shall also be given to
Jonathan C. Bevins, with proof thereof provided at the hearing.
