1: I, Earl H. Kean, am an adult resident of the Commonwealth of Pennsylvania domiciCed T
at 231 Dogwood Drive, Elizabethtown, Lancaster County, Pennsylvania.
2. I am one of three sons born to the late George W. Kean and the late Mary Kean. The
other two sons are my late brother, Gary Kean and my late brother, Jason A. Kean.
3. George W. Kean died having attained the age of 69 years at 0740 hrs. on March 24,
2000 at 710 Marden Avenue, Shippensburg, Franklin County, Pennsylvania. At the
time of his'death George was'a resident at 710 Marden Avenue, Shippensburg, Franklin
County, Pennsylvania. Affidavit of death of George W. Kean is attached and
incorporated by reference.
4. Mary Kean died having attained the age of 73 years at 2115 hrs. on August 1, 2005 at
2344 Deny Street, Harrisburg, Dauphin County, Pennsylvania. At the time of her death
Mary was a resident at 854 Woodridge Drive, Middletown, Dauphin County,
Pennsylvania. Affidavit of death of Mary Kean is attached and incorporated'by
reference.
5. Gary Kean died having attained the age of 52 years at 1052 hrs. on December 1, 2008
at 157 North East Street, Carlisle, Cumberland County, Pennsylvania. At the time of
his death Gary was a resident at 157 North East Street, Carlisle, Cumberland County,
Pennsylvania. Affidavit of death of Gary Kean is attached and incorporated by
reference.
6. Gary Kean died with issue, a daughter Ashley Allison Kean, DOB August 8, 1982-.
7. Jason A. Kean died having attained the age of 48 years at 0534 hrs. on July 30, 2014 at
Hershey Medical Center, 500 University Dr., Hershey, Dauphin County, Pennsylvania.
At the time of his death Jason was a resident at 6111 Haymarket Way, Mechanicsburg,
Cumberland County, Pennsylvania. Affidavit of death of Jason A. Kean is attached and
incorporated by reference.

3. George W. Kean died having attained the age of 69 years at 0740 hrs. on March 24,

Kean was interred.

L 1, Earl H. Kean, am an adult resident of the Commonwealth of Pennsylvania domiciled at
231 Dogwood Drive,Elizabethtown, Lancaster County,Pennsylvania.
2. 1 am the son of the late Mary Kean.
3. Mary Kean died having attained the age of 73 years at 2115 firs. on August 1, 2005 at
2344 Derry Street,Harrisburg, Dauphin County, Pennsylvania. At the time of her death
Mary was a resident at 854 Woodridge Drive, Middletown, Dauphin County,
Pennsylvania.
4. On or about August,20051 was made personally aware that the cremains of the late
Mary Kean along with a container identifying the contents as the cremains were placed
upon the gravesite of the late George W. Kean Lincoln Cemetery in Chambersburg,
Franklin County, Pennsylvania. This information was conveyed to me by my cousin,
Shirley K. Jones of Chambersburg, Franklin County, Pennsylvania.
I, Earl H. Kean,being duly sworn according to law,deposes and says that the facts set forth in
the foregoing affidavit are true and correct to the best of my knowledge, information and belief.
