ORDER
And now this � .� day of , 2014, upon consideration of
the within the Petition for Court Approval of Settlement of a Death Case Involving Wrongful
Death And Survival Claims, it is hereby Ordered that the settlement in the total amount of
$33,333.33 is approved. Counsel fees and expenses are found to be fair and reasonable and they
are approved as set forth below. Distribution is directed as follows:
1. To be paid to Metzger, Wickersham, Knauss & Erb, P.C. as attorney's fees 25%
of the gross settlement amount, or $8,333.33.
2. To be paid to Metzger, Wickersham, Knauss & Erb, P.C. the sum of$492.26 as
reimbursement of expenses.
3. To be paid to Fogelsanger-Bricker Funeral Home as reimbursement of funeral
expenses the amount of$7,7711.34.
4. To be paid to the Commonwealth of Pennsylvania Department of Public Welfare
the sum of $12,253.87 which represents 50% of the settlement proceeds after deduction of
attorney's fees, less 50% of the pro rata share of costs. � � �

5. The balance of$4,482.53 shall be deposited in a federally insured and restricted
savings account or certificate of deposit in the name of Elisa J. Perry, a minor, to be marked that
no sum shall be withdrawn until Elisa J. Perry obtains majority, except as authorized by prior
Order of Court. Proof of said account with restrictions thereon to be filed with the Court within
thirty (30) days of receipt of the proceeds.
6. Petitioner is authorized to execute an appropriate release reflecting the settlement.
