The Petitioner herein is SULIN BRENAMAN, also known as SU LYNN BRENAMAN, an adult individual who currently resides at 2020 Stumpstown Road, Mechanicsburg, Upper Allen Township, Cumberland County, Pennsylvania. 2. Petitioner was born on the 21"day of June, 1947 at Hanover General Hospital in Hanover, Pennsylvania. The Hospital Birth Certificate issued at the time listed Petitioner's birth name as SU LYNN BRITCHER. 3. Petitioner was united in marriage with her then and current husband, PAUL A. BRENAMAN, at Grantham, Pennsylvania on the 3rd day of August, 1963 under the name of SUE LYNN BRITCHER, all per the marriage license issued by the Clerk of the Orphans' Court of Cumberland County, Pennsylvania docketed as No. 447, Volume 59. 4. Petitioner's official Certification of Birth issued by the Vital Records Division, of the Department of Health, of the Commonwealth of Pennsylvania, on the 19th day of May, 2014, State file No. 123392-1947 lists Petitioner's name as SU.LYNN BRITCHER. n d f7" A.,._ . r _1 5. Social Security Administration Records list Petitioner's name as SUE BRITCHER BRENAMAN. 6. Petitioner's Driver's License lists Petitioner's name as SULIN BRENAMAN. 7. Petitioner believes and avers that the spelling on her original birth certificates as "SU" was a typographical error and that her name should actually have been and was intended to be spelled as "SUE." 8. g Petitioner, in common usage and social intercourse has always referred to herself as and used the commonly accepted spelling of"SUE" except on occasions when she has been told that she must use"SU" for official purposes. 9. Petitioner's proper middle name is "LYNN" although the spelling of"LIN" has now been occasionally appearing on official items from time to time. 10. Petitioner and her said husband have resided at the 2020 Stumpstown Road, Mechanicsburg address for thirty-one (3 1) continuous years prior to the date of filing this petition. 11. Petitioner and many of those who deal with her on an official basis have been and continue to be subject to manifest confusion often resulting in delays, requests for additional proofs and documents, and generally unnecessary assorted additional burdens and aggravations. 12. Petitioner therefore respectfully requests your Honorable Court to change her name officially to "SUE LYNN BRENAMAN." 13. Petitioner has no criminal record. 14. There are no judgments or other adverse decrees entered against Petitioner. 15. Petitioner does not seek a change in her name to confuse or to avoid creditors, to avoid debts, or to avoid any other obligations or responsibilities. 16. Petitioner's spouse is in agreement with this Petition and will so state at the hearing.
