guardian by Order of the Court dated July 23, 2014. _ N c� �
2. The incapacitated person is living at The Episcopal Home in Shippensburg, PA.
3. This report constitutes the filing of the Initial Report.
4. Listed below are the sources of income for Ms. Cline on a monthly basis:

much in disrepair. Melvin Cline has been admitted to Claremont Nursing and
Rehabilitation on a permanent basis.

The monthly charge for The Episcopal Home $2357.00. This is paid by the
spouse but it was agreed that $500 will be paid to the spouse's account.
The $62.00 received as a pension is automatically applied to the Unsecured
Loan at Members 1St by the credit union. The remaining amount for the loans are
being paid by her spouse. Currently, no payments are being made on the credit
cards.
9. The incapacitated person receives monthly social security benefits directly. The
Cumberland County Aging and Community Services will be applying to become
the representative payee for her social security benefits.
