Walden, being duly sworn according to law, does hereby declare: 1 The Decedent, Elizabeth Bulota, utilized PNC bank for all her ordinary and day to day banking matters and maintained investment accounts with Merrill Lynch. 2. At the time of her passing she transacted her banking at a PNC bank branch located in Bethany Village-her residential community at the time of her passing. 3. Decedent had maintained a safe deposit box at the Bethany Village branch which was closed in August of 2013. Nonetheless, a PNC safe deposit box key was found in the decedent's residence. The key was used for the box in question but the box was empty. 4. The decedent had not used Merrill Lynch for any day to day banking services since the passing of her late husband, Stanley Bulota, more than eight (8) years prior. Merrill Lynch was aware of the lost Will, but did not offer safe deposit box services, and had no knowledge as to the disposition of the decedent's original Will. 5. The decedent, along with her son, Stephen Walden, and daughter, Gail Coleman, opened a safe deposit box with Metro Bank in Mechanicsburg, Pennsylvania approximately six (6) years ago but the box was closed approximately six (6) months thereafter. 6. The undersigned performed a thorough search of the decedent's residential unit and all her records and paperwork, as did the decedent's daughter, Gail Coleman. 7. During the last year of decedent's life, her daughter, Gail A.Coleman, assisted her with her bills, paperwork and record keeping. The decedent never mentioned to her daughter any changes to her Will or related planning and documentation. 8. No family member of decedent ever heard decedent discuss any further changes to her dispositive scheme or documentation. 9. The decedent conferred with her estate planning counsel, Thomas E. Flower, Esquire in May of 2013 and at that time confirmed her intended disposition consistent with her Will of April 14, 2003, but discussed a possible TOD designation for an account at Merrill Lynch containing proceeds of an inheritance she received from her deceased sister which were deemed separate from the assets which were the subject of the Marital Agreement for Mutual Wills which is referenced in the undersigned's Petition and attached thereto.
