Petitioner is the natural father of Juliette, whose date of birth is January 9, 2007. The natural mother of Juliette, Amy Vezina, passed away on January 16, 2014. 3. Petitioner has been a resident of Cumberland County, Pennsylvania for at least six (6) months previous to the filing of this Petition. 4. Since birth Juliette has resided at the following addresses: a. From birth to September 1, 2010, with Petitioner, Amy Vezina, Amy Vezina's daughter, Cassidy Vezina, and Juliette's sisters, Seneca Mary Vezina and Mabel Jean Vezina, at 28 Payne Street, Springvale, Maine; b. From September 1, 2010, to January 16, 2014, with Petitioner, Amy Vezina, Amy Vezina's daughter, Cassidy. Vezina, and Juliette's sisters, Seneca Mary Vezina and Mabel Jean Vezina, 115.75- pel 0#fr36/3 3 R4- 31,2 6-6 r% at 302 S. Pitt Street, Carlisle, Pennsylvania. 5. Petitioner desires to change Juliette's name from Juliette Marie Vezina to Juliette Marie Vezina Sheesley. 6. Petitioner desires and feels it would be in the best interest of Juliette that her name be changed for the following reason: a. Petitioner desires that Juliette's last name conform to Petitioner's last name. b. Petitioner desires to have Juliette's name continue to reflect the last name of her mother, Amy Vezina. 7. Petitioner does not wish to change Juliette's name for any illegal purpose, to avoid the payment of taxes or other debts, or to avoid any obligation. 8. No judge -has been assigned to this matter.
