Petitioner is the natural father of Seneca, whose date of birth is March 12, 2002. The natural mother of Seneca, Amy Vezina, passed away on January 16, 2014. 3. Petitioner has been a resident of Cumberland County, Pennsylvania for at least six (6) months previous to the filing of this Petition. 4. Since birth Seneca has resided at the following addresses: a. From birth to March 24, 2003, with Petitioner, Amy Vezina, and Amy Vezina's daughter, Cassidy Vezina, at 46 Walnut Street, Carlisle, Pennsylvania; b. From May 1, 2006, to January 9, 2007, with Petitioner, Amy Vezina, Amy Vezina's daughter, Cassidy Vezina, and Seneca's sister, Mabel Jean Vezina, at 28 Payne Street, Springvale, Maine; d. From January 9, 2007, to September 1, 2010, with Petitioner, Amy Vezina, Amy Vezina's daughter, Cassidy Vezina, and Seneca's sisters, Mabel Jean Vezina and Juliette Marie Vezina, at 28 Payne Street, Springvale, Maine; e. From September 1, 2010, to January 16, 2014, with Petitioner, Amy Vezina, Amy Vezina's daughter, Cassidy Vezina, and Seneca's sisters, Mabel Jean Vezina and Juliette Marie Vezina, at 302 S. Pitt Street, Carlisle, Pennsylvania; and f. From January 16, 2014, to present with Petitioner, Amy Vezina's daughter, Cassidy Vezina, and Seneca's sisters, Mabel Jean Vezina and Juliette Marie Vezina, at 302 S. Pitt Street, Carlisle, Pennsylvania. 5. Petitioner desires to change Seneca's name from Seneca Mary Vezina to Seneca Mary Vezina Sheesley. 6. Petitioner desires and feels it would be in the best interest of Seneca that her name be changed for the following reason: a. Petitioner desires that Seneca's last name conform to Petitioner's last name. b. Petitioner desires to have Seneca's last name continue to reflect the last name of her mother, Amy Vezina. 7. Petitioner does not wish to change Seneca's name for any illegal purpose, to avoid the payment of taxes or other debts, or to avoid any obligation. 8. No judge has been assigned to this matter. WHEREFORE, Petitioner, Benjamin S. Sheesley, respectfully requests this Honorable Court to issue an Order changing his daughter's name from Seneca Mary Vezina to Seneca Mary Vezina Sheesley.
