A true and correct copy of the executed oath of subscribing witness is attached hereto as Exhibit"A" and incorporated herein by reference. 4. I contacted her former employer, the Krueger Law Firm regarding the whereabouts and contact information for the second subscribing witness, Dilma Felgueiras. 5. Mr. Krueger informed that his office has not had any contact with Dilma Felgueiras for several years and that she had moved out of state. A true and correct copy of the email correspondence from Richard Krueger dated October 21, 2014 is attached hereto as Exhibit"B" and incorporated herein by reference. 6. Our office was unable to locate the whereabouts of Ms. Felgueiras or any contact information for her.
