AND NOW, comes Cassandra Jane Kinsey, by and through her Attorney, Jane Ad s,
Esquire, and respectfully requests this Honorable Court to change her name to Cassandra ight
Kinsey, for the following reasons:
I. Petitioner, Cassandra Jane Kinsey, (hereinafter referred to as "Petitioner") curren y
lives at 1]40 N. West St., Carlisle, Cumberland County, Pennsylvania, ] 7013.
2. Petitioner's date of birth is May 4, 1987,
3. Petitioner lives with her natural mother, Joanne Kinsey, and her natural father, C 011
Kinsey.
4, Petitioner desires to be known by the name of Cassandra Knight Kinsey, since Kni ht
is a family name.
5, Petitioner has no outstanding credit debt that could be avoided through a name ch ge,
6, Petitioner has no outstanding federal or state tax debt that could be avoided through a
name change,
7, Petitioner's parents are in agreement with her request.

WHEREFORE, Petitioner requests that this Honbrable Court Order that her name is
changed to: Cassandra Knight Kinsey.
