AFFIDAVIT OF NIKOLAS A. ZULLI IN SUPPORT OF AN ORDER APPOINT l'NG
DONALD N. ZULLI, SR. AS GUARDIAN FOR DONALD M. ZULLI, JR.
I, Nikolas A. Zulli, being duly sworn according to law, depose and say the
following:
1. My name is Nikolas A. Zulli, and my address is 57 South East Street,
Apartment 2, Carlisle, PA 17013.
2. 1 am the sole adult child of Donald M. Zulli, Jr., an alleged incapacitated
person.
3. 1 have been duly notified that the Court may deem it necessary to appoint a
Guardian to represent the interests of my father, Donald M. Zulli, Jr., an
alleged incapacitated person.
4. 1 have been duly notified that my Grandfather, Donald M. Zulli, Sr., wishes to
serve as Guardian for my father, Donald M. Zulli, Jr., an alleged incapacitated
person, should the Court deem it necessary to appoint an individual to that
,position.
5. Should the appointment of a Guardian be deemed necessary by the Court, I
hereby affirm that I support the appointment of Donald M. Zulli, Sr., to serve
as Guardian for my father, Donald M. Zulli, Jr., an alleged incapacitated
person.
6. 1 represent that the facts set forth herein are true and correct to the best of my
knowledge, information and belief.
