Adams, Jr., is an adult individual whose current address is 916 Gobin Drive, Carlisle, PA, 17013, and who serves as the Executor of the estate of Betty L. Adams, his mother, who died on January 10, 2014. 2. Petitiorier previously filed a Petition for Settlement of a Small Estate and a Petition to Make F:ule Absolute, thereby allowing the within estate to be settled by Order dated October 28, 2014, entered pursuant to said Petition, a copy of said Order being attached hereto an�� incorporated herein by reference as Exhibit"A." 3. Petitiotier is in receipt of the check dated March 12, 2014, from Highmark Blue Shield in the amount of $724.76 payable to `B Adams," which reimbursement check was previouisly identified in the Petition for Settlement of a Small Estate, a copy of said check being attached hereto and incorporated herein by reference as Exhibit"B". 4. Petitioner is in receipt of a check from the United States Treasury dated April 1, 2014 in the amount of $500.00 payable to "Richard Adams Adm, Betty L Adams Decd," which check was previously identified as a personal income tax refund check in ��_ cr� c::� <;� r� �.��.,� .�....��. As per the filing of the Petition for Settlement of a Small Estate, this is an insolvent estate and the deposit or distribution of the above referenced checks to your Petitioner, who has advanced his own funds to pay costs and fees of the estate, does not affect the fact that this is an insolvent estate. 6. Pursuant to Pa.C.S. §3101, Petitioner is requesting the issuance of an Order of Court authorizing him to endorse and receive distribution of the above referenced checks. WHEREFORE, Petitioner requests your Honorable Court to issue an Order authorizing him to endorse the check from Highmark Blue Shield and from the United States Treasury, through any bank or financia] institution of Petitioner's choosing, and receive and retain distribution of the proceeds from the cashed checks.
